Eligibility
Dental and vision patients of LevelUp Kids must meet income criteria set by the U.S. Department of Health & Human Services to receive our services. We accept patients whose households fall at or below 200% of the 2024 Federal Poverty Levels:
1 person household = $30,120
2 persons per household = $40,880
3 persons per household = $51,640
4 persons per household = $62,400
5 persons per household = $73,160
6 persons per household = $83,920
7 persons per household = $94,680
8 persons per household = $105,440
9 persons per household = $116,200
For families who are enrolled in Medicaid, we will bill Medicaid. To better assist our patients, we have added some helpful resources when applying for Medicaid. Below, we have listed links from the Missouri Department of Social Services website. If you do not have access to the internet, we can provide paper applications at our office.
